Tonya is a 32-year-old woman who will be seen across the pregnancy continuum from antepartum to intrapartum through NB stabilization. This case has a special focus on identifying and discussing SDOH for UG nursing students enrolled in a women’s health course. During each phase the student team will complete an assessment, identify relevant SDOH, and discuss ways to improve health and wellness for the patient.
Learning Objectives
1. Perform a health history on initial evaluation at the OB Clinic using 2-3 therapeutic communication strategies.
2. Review patient’s current living conditions and identify SDOH that are impacting health, wellness, and access to social services.
3. Utilize therapeutic communication to create an opportunity to complete a health history, develop trust, and open lines of communication between the healthcare team and the patient.
4. Perform complete nursing assessment of patients in the OB clinic, including history and physical exam.
5. Perform nursing assessment of patients in the intrapartum setting, including review of fetal well-being and labor progress based on current assessment of labor.
6. Perform initial newborn stabilization and nursing assessment of newborn patient in the immediate post-delivery setting.
7. Synthesize how a patient’s SDOH factors impact discharge planning through open communication and correctly identify patient wishes in her treatment plan.
8. Offer local resources to assist in patient care and transition to home by identifying strategies aimed at improving the health and wellness of diverse patient groups.
